Overview
This original research article is a descriptive cross-sectional study of 80 patients who underwent total laparoscopic hysterectomy (TLH) for benign disease at a teaching hospital in Anuradhapura between September 2020 and October 2023. It documents how the technique was established and refined over a single surgeon’s first 80 cases in a rural Sri Lankan setting.
Key findings
The most common symptomatic indications were secondary dysmenorrhoea, heavy menstrual bleeding and chronic pelvic pain.
Adenomyosis and fibroids were the leading underlying pathologies.
Mean operating time was approximately 111 minutes.
Operating time correlated positively with uterine weight and a history of previous abdominal surgery.
Why it matters
The series demonstrates that total laparoscopic hysterectomy can be delivered safely and reproducibly outside major metropolitan centres, supporting wider adoption of minimally invasive gynaecological surgery in Sri Lanka’s public health system.
